         GUIDELINES FOR INCOMING GRADE 7 APPLICATION FOR ADMISSION
                                      (School Year 2020 - 2021)

1) Applicant for incoming Grade 7 Admission Test must have the following 
      qualifications:
  a. A Filipino citizen and a bona fide resident of Valenzuela City. For applicant
       under guardianship, applicant must have completed Grades 5 and 6 in any
       public or private school in Valenzuela City;
  b. With average grade of at least 85 in ALL SUBJECTS for the first two quarters
       in Grade 6 and NO grade LOWER than 80; and
  c. Physically fit and of good moral character.
2) Qualified applicant may submit the following requirements beginning on
      November 18, 2019 until January 10, 2020 (9:00 AM – 4:00 P.M., at the Office
      of the School Registrar):
  a. Duly accomplished Admission Test Application Form;
  b. Student’s Report Card (Form 138-A) with first two quarter grades certified by
       the School Principal/Registrar;
  c. Proof of Parent/Guardian Residency (Voter’s ID and proof of billing, i.e. Electric,
       Water or Phone bills);
  d. Omnibus Certification on authenticity of documents (duly notarized); and
  e. Notarized Affidavit of Guardianship for applicant under guardianship.
3) Application form is now available in the Office of the Registrar or downloadable
      from the school official website http://valscience.webs.com/admission
